People may not realize this, but right clicking in the spell section of the Birthsign menu only gives you one option: "Delete". For all those wondering how to actually add a spell, you simply drag-n-drop any spell you want. It could be one to summon a lovable sheep, or one that makes you character god-like. If you want to delete a spell that is already there, right-click it and press delete. Any spell can be added to a birthsign, except ones that damage health to the player are not recomended.

When creating a new birthsign, sometimes a birthsign will not show up in game, or will not be selectable in the birthsign selection screen in game. The workaround for this is to create several copies of the birthsign and then enter the game to see which one shows up.
